Download Application form to Change your Picture

If you want to change your picture in AIOU records you may request by using Application form only if your picture is wrong by mistake or if your photo is not availabe on your Roll Number slip.

After download form from above link, print it out, fill it carefully and send it to given address :

Data Processing Manager,

Computer Center, Block Number # 3,

Sector H-8 , Allama Iqbal Open University, Islamabad

Note that there is no need to attached any addtional application with this form.

Important Instructions :

• Picture should be passport size (1.5″ X 2.0″)
• Do not use pins or stapler to attched pictures only use Glue or gum
• Only one picture should be attested from front side

These instructions are also availabe on the form in Urdu. You need Two (2) pictures to attached with the form, one should be attested and other attached without attested.
